Forms/Survey Engine

The Forms/Survey Engine is an internal company tool that allows you to custom design forms for any of your company’s feedback needs.  It lets you phrase and input your own questions, make changes and even test drive the form before sending it out.  Site Administrators must grant security rights in order to use this tool.
