Survey Format

The Forms Engine has two options for formatting each form or survey:

Flash

HTML

To choose the format, select the Survey Format drop down menu.

HTML:	If HTML format is selected, users will be able to print the form/survey directly from CSI Secure Connect.
Flash:	If Flash format is selected, users can choose different options for how the form/survey is displayed.
